The Cattle Killing, by John Edgar Wideman

Set in the late-19th century in a village outside the burgeoning city of Philadelphia, which is recovering from the plague. The book is narrated by a black preacher called "Eye" or "I" who relates episodes of his life to a woman he is trying to heal.
